Here at Country Meadows Clinic our priority is YOU! We want you to get the best care possible and want to respect your time by keeping our schedule in order. We want to remind everyone that when making an appointment please understand that appointment times are scheduled based on the reason for visit. These times come in 15 minute increments. Depending on the reason, scheduling may be for 15 minutes up to a 45 minute time slot. We work hard to stay on schedule but need our patients help to do so. Some appointments/providers will not have online availability, so please call the office to schedule if you do not see your specific appointment type. Please be sure to schedule with your assigned primary care provider.
Elation Passport is our patient portal that lets you keep in touch with our office between visits, so that all of your visit takeaways, lab results, and recommended healthcare steps are captured in one place. You can also access your complete clinical information like allergies or medications for use as a reference.
We care greatly for those patients who have the inability to leave home for clinic visits. Certain Medicare patients will qualify for home visits. Call to see if you or your family qualify for this service.
We ask that you arrive 10-15 minutes prior to your appointment time.
If you are more than 15 minutes late to your appointment, you may be asked to reschedule.
Patients arriving past the appointment window time along with missed appointments may be charged a $35 fee. Cancellation requires 24 hours advance notice.
We will not be providing pain management services at Country Meadows Clinic. This means, if you get any narcotic pain medication on a monthly basis you will need to get a referral or schedule with a pain management provider
